a. *如果申请人的家庭地址跟ID上显示的不一样。请提供现有家庭地址证明;

3. 银行支票（Void Check）

a. *如果没有正式支票， 请叫银行人员写一封证明信：包含账户号码和银行的

代码， 还有申请的日期。如果DBA跟公司名字不一样， 请用DBA;

4. 店名跟公司名， 地址， 电话;

The following documents are needed for merchant who want to accept credit

card payment:

a. *you will need an address proof if your current address is difference to your state ID;

Void Check from your business checking account;

a. *if you’re not able to provide a void check, you will need to ask your bank to issue a

bank letter to with your company name, full routing number, full account number

and account opening date on it. If your DBA name is different to your company

name, please put DBA name instead;
